Our Approach
Bethesda Project operates a continuum of care throughout Center City Philadelphia utilizing a Housing First approach that minimizes barriers to access and allows each individual to take their own journey of healing at one's own pace. Individualized Case Management
Each guest and resident completes an Individualized Care Plan with their own case manager, which allows them to identify goals they wish to achieve. With the Care Plan in place, case managers and engagement specialists connect guests and residents to services in the community that assist in achieving their goals.
The overall goal of Bethesda Project's case management services is to assist its guests and residents in becoming more stably housed and increase their overall independence.
Bethesda Project’s Theory of Change
The Theory of Change (ToC) grew out of Bethesda Project’s participation in Pew Foundation’s Evaluation Capacity Building Initiative (ECBI) in 2021 - 2022. The ToC clarifies the ultimate impact we seek to achieve: ending chronic homelessness among single adults in Philadelphia, while also identifying the short, medium, and long-term outcomes that arise from our core programming components: person-centered services, low-barrier shelter and housing, and system and community engagement. The work to develop an organization ToC was spearheaded by the EBCI Team, which included staff at the director, supervisor, and case manager levels. Each facet of the ToC was shared with leadership staff for feedback and further refinement. Moving forward, the ToC will serve as a foundation for our measurement, evaluation, and learning activities. For more information on our Theory of Change, click here.
